If this is your first online Public Records Request, you are encouraged to create an account by clicking “Create Account.” By creating an account, you will have the ability to track and monitor your public records request.

You may also submit a public records request anonymously. Please note that if you submit an anonymous request, you will need to retain your Confirmation Number to view or pick up your documents online or at City Hall. Please see further instructions below.

You will not be able to receive updates or track the progress on the request submitted.

If you do not create an account, it will be your responsibility to maintain your Confirmation Number (ex. P002553-093015). Frequently check the Public Records Center for updates and to request additional information or clarifications. If you lose your confirmation number, you will need to submit a new request. To use this option, click “Submit” below. To search for a previously submitted request, click on the “My Public Records Center” link at the top of the page and then click “Search by Reference Number”.

The Police Department cannot communicate directly with you if your contact information is not provided.

GovQA is an SFPD authorized third party vendor which facilitates incident report request services for CISU. SFPD accepts no liability for the accuracy of content on the linked GovQA webpages. Members of the public may still make requests using U.S. mail or email by using the Incident Report Request Form(s). Those requests will be manually entered and tracked in the Incident Report Request Portal by a member of CISU. An account on the Incident Report Request Portal can be created to track the request online.
